Pfizer discontinued development of its hemophilia B gene therapy less than a year after approval, citing limited market interest. The decision follows a broader pattern of companies withdrawing from adeno-associated virus vector-based therapies. Meanwhile, Bluebird Bio, once valued at $10 billion, recently sold for under $30 million after treating only about 50 patients across three approved gene therapies.
The financial barriers extend beyond manufacturer struggles. Current gene therapies for sickle cell disease cost between $2 and $3 million. Medicare approved add-on payments reaching 2.3 million dollars for one product and 1.65 million for another in fiscal year 2025. These figures exclude pretransplant evaluation, stem cell collection, the transplant procedure itself, fertility preservation, and follow-up care.
Treatment complexity adds to payer concerns. Patients undergo months of evaluation, stem cell collection requiring 60 to 90 days for modification, myeloablative chemotherapy, approximately two months of hospitalization, and extended recovery. The Centers for Medicare and Medicaid Services launched a voluntary outcomes-based payment model to address access barriers while managing costs.
Advanced techniques for bone marrow transplantation now offer compelling alternatives at roughly half the expense. Reduced-intensity conditioning, haploidentical donors, and post-transplant cyclophosphamide have increased two-year survival rates above 90% while reducing graft-versus-host disease to under 5%. These advances raise the threshold of value that gene therapies must meet to justify their premium pricing.
