A STAT News investigation raises pointed questions about the expanding financial ties between pharmaceutical manufacturers and telehealth platforms, and what those arrangements mean for prescribing integrity and plan spending.

The model works as follows: drugmakers partner with telehealth companies, supply discount coupon codes, and direct patients to branded consultation portals where a brief survey — not a clinical encounter — typically ends in a prescription. Sprout Pharmaceuticals, maker of the low-libido drug Addyi, offered $10 visits through its partner, Prescribery, using promotional codes distributed via social media. Similar arrangements exist for the weight-loss drug Contrave, a prescription digital therapeutic for depression, and an eyelid-lifting eyedrop whose telehealth vendor is part of the same corporate family as the manufacturer.

For managed care organizations, the implications are significant even when these visits are structured as cash-pay, deliberately outside federal health program coverage. Critics, including health law experts at Harvard, Brown, and Penn, argue the arrangements create financial architecture designed to move patients toward specific branded drugs quickly and cheaply, bypassing the diagnostic thoroughness that might point toward generics, behavioral interventions, or no treatment at all. The result can be uncoordinated care that ultimately affects the total cost of care when those patients later interact with their plans.

Federal anti-kickback statutes may not reach these arrangements precisely because they are engineered to avoid federal program involvement, a compliance posture health policy researchers say warrants closer scrutiny from regulators and payers alike. As telehealth access expands under value-based and supplemental benefit structures, plans should weigh whether low-barrier, pharma-linked telehealth portals are consistent with utilization management and formulary stewardship goals.
