The Deprescribing Dilemma

by Sonia Reyes - Last Updated: Sep 13, 2026

Dr. Randy Vogenberg and Tamara Thomas explore the challenges and gaps in managing polypharmacy across healthcare systems. He explained how polypharmacy has evolved from primarily affecting senior populations in institutional settings to now impacting aging-in-place patients, with multiple prescribers creating communication gaps and a lack of clear responsibility for deprescribing processes. He further explained that while state and federal regulations exist for prescribing, standards for deprescribing are minimal, leading to what Tamara called a "wild wild west" approach in community settings. The discussion addressed technical challenges including digital data interoperability between different healthcare systems and electronic health records, with Randy suggesting that, ideally, AI could facilitate better data sharing and patient-centric approaches if properly implemented.
